This is a unique program extended to Upper Deck Authenticated collectors that provides the opportunity to have personal items autographed by UDA�s exclusive athletes.

All items will be subject to UDA�s patented five-step process
A special �Exclusive Signature Service / Show� hologram will be placed on all items and will match the hologram on the accompanying certificate of authenticity

The Process
If you�re interested in sending in an item to be signed, please follow the process below:

1. Customer must provide images of the product to UDA to receive
approvals

2. Once your item has been approved, a unique Customer
Identification number will be assigned to your piece and recorded
within the Upper Deck database.

3. Mail in your item to Upper Deck with your Customer ID number
clearly marked on the box (sorry but no piece will be accepted
without your Customer ID code).

4. Once your item is signed, it will take approximately 2 weeks to get your collectible mailed back to you.

THE NUMBER OF PERSONAL ITEMS ATHLETE SIGNS MAY BE LIMITED

The eligibility of the *personal items will be determined by
The Upper Deck Company, LLC (�Upper Deck�) and Athlete.

All mail-in items are sent at customer�s risk. Any mail-in items received after the delivery date will be returned to customer. Price include shipping, handling and insurance on the return of your item

Upper Deck reserves the right to determine pen color & signature location on all ESP items

Upper Deck and its agents are not responsible for lost, late, damaged, misdirected, incomplete, illegible, stolen or postage due mail-in items

Under no circumstances will Upper Deck have any liability in excess of $200 per item
Please allow 2-3 weeks following the signing for shipping

There are NO refunds or exchanges on mail-in items received by Upper Deck

Items previously signed by other athletes will not be accepted, unless approved by our UDA spokesmen
